Hawthorne Blvd., #305 -- on the west side of Hawthorne
Blvd., just south of Carson St. in Torrance, California (across from the Del Amo Mall). Ample parking is available in the
back. Elevator available to 3rd floor suite. On your first visit, allow 90 minutes for the consultation and treatment. Arrive
a few minutes early to fill out new patient paperwork, or request to have it faxed or sent to you ahead of time. Bring your insurance card. It is helpful also if you bring a list of medications/vitamins/supplements and any pertinent medical reports such as recent MRI's, Xrays, etc. BILLING/INSURANCE Cash or check accepted only, no credit cards. Accepted: Private or company-paid insurance, worker's comp, personal injury If your plan allows benefits for acupuncture, we will take care of billing for you. Julee Haar Chui is considered an in network provider for the following major insurance companies: Blue Shield • Great-West Healthcare • Pacificare • United Healthcare and an out of network provider for the following major insurance companies: Anthem Blue Cross • Aetna • many others, please call for more information. Note: Medicare does NOT cover acupuncture, but if patient is Medicare eligible, an extra discount may apply.
